Sicily, the largest Mediterranean Sea island, is just off the "toe" of Italy's "boot." Its rich history is reflected in sites like the Valley of the Temples, the well-preserved ruins of 7 monumental, Doric-style Greek temples, and in the Byzantine mosaics at the Cappella Palatina, a former royal chapel in capital city Palermo. On Sicily’s eastern edge is Mount Etna, one of Europe’s highest active volcanoes.

- Nicky Farrugia completed the first swim from Sicily, Italy to the island of Gozo in the Malta archipelago in 1985 in 30 hours 17 minutes.
- Neil Agius completed a 100 km Sicily to Malta swim in 28 hours 9 minutes on 26 June 2020.
